The Dr. Robert E. Appleby School Based Health Centers provide on-site, medical and mental health services to those who need it the most – children and adolescents – when they need it. The SBHCs are the only organization in Norwalk that provides no out of pocket cost for any of our services, as well as the only agency in Norwalk that provides onsite school based medical and mental healthcare. The SBHCs are licensed as outpatient clinics (Public Health code Sections 19-12DA45 through 19-13-52).

The purpose of SBHCs is to provide a model of service delivery that ensures comprehensive care to children and adolescents improving the overall physical and emotional health of students in order to maximize their educational experiences and potential. In addition, the availability of on-site healthcare reduces absenteeism and encourages adolescents to take responsibility for their own well-being.

SBHCs serve students (grades 9th – 12th) in the three Norwalk high schools: Norwalk High School, Brien McMahon High School and Norwalk Pathways Academy; as well as students (grades 6th – 8th) who attend Nathan Hale, Ponus Ridge, and West Rocks Middle Schools. The SBHC staff includes Nurse Practitioners, Physician Assistants, Licensed Clinical Social Workers, Office Administrators, part-time Pediatrician, Adolescent Psychiatric Consultant and Program Director. Our SBHC professionals work closely with school personnel, such as the nursing, social work and guidance departments.

Goals

  • Provide diagnosis and treatment of acute illnesses and injuries
  • Provide preventative services, including outreach, family support and health education
  • Assist adolescents in adopting behaviors that promote their good health and well-being
  • Assist adolescents in reducing morbidity/mortality through early identification of health issues and problems

  • Diagnosis and treatment of acute and chronic illnesses (step throat, bronchitis, ear infections, asthma)

  • Health education, risk reduction and health promotion
  • Referral and follow-up for specialty care

Counseling Services

  • Crisis intervention
  • Psychiatric and medication assessments
  • Individual, group and family therapy focusing on depression, anxiety, adjustment problems, eating disorders, alcohol, tobacco and drug use

  • Services are available to all students who have a signed permission slip on file
  • Forms are available at each health center or The Human Services Council
  • There is no out-of-pocket charge for any service that is provided
  • All information is kept confidential (except in extraordinary circumstances, such as threat to self or others)

We are proud of our numbers! Over the past school year, the School Based Health Centers enrolled 2,200 students, provided over 7,500 medical or mental health care visits, and were able to return 92% of students back to class on the same day.
